Registration
Registration was administered using an online Registration Form which gathered some preliminary information about the participant, and reserved a space. Registrants for the sessions included providers, consumers and educators from across Canada.
To access the audio recordings of all sessions and other program materials, visit the Resources page.
Instructions for completing the Registration Questionnaire
- Click on the icon to access the online survey.
- It will take you about 10 minutes to complete the survey (about 11 screens). Use the "next" button to move through the survey.
- After you complete the survey, you can choose to go back through your survey responses (using the back button on your Browser) and print each screen using your browser menu options.
- Be sure to click on "Done" at the end of the survey to automatically return to this website and receive a "Thank you for registering" message.
